This report should be filled out after any contest where an ejection occurs or unusual situation develops.
The situation could occur prior to, during or after the contest.
Each reported situation will be followed up by the TSSAA.
PLEASE SEND THE REPORT NO LATER THAN 8:00 AM THE DAY FOLLOWING THE CONTEST.
